How to Make a Referral
By Parents: Please call your child’s Service Coordinator (SC) and request a need for the training; your SC will guide you appropriately. If your child does not have a diagnosis and you are not sure, email us or give a call.
By Service Coordinator:
- Have the desired outcome determined and write an Individualized Program Plan goal
- Referral packet should have the following:
One-page profile of the child / Individualized Program Plan (IPP) goal / face sheet / consent. Download and print our referral fax cover sheet, then fax all information to (805) 384-0988.
Parents will be contacted within one week for service.
Initial report will be sent to SC within two weeks of commencement of service.
This is a time-limited service which is intended to be completed within six months. However, it is preferable to keep the authorization open for 12 months to avoid date extension if parents were not available as determined.
